CPR: Rescue breaths using a bag-mask device

Equipment for the practical skill:
- protective medical gloves;
- СPR simulator;
- bag-mask device.
Clinical situation:
While at the train station, you heard a call for help. As you approached the scene, you found a group of people surrounding a woman (appearing to be about 60 years old) lying on the floor. First responder beside the casualty asks you to bring a bag-mask device from medical station. You delivered the device to the scene of the incident. First responder is performing chest compressions. You should give rescue breaths using the bag-mask device.
Order of the skill performance:
- Prepare the breathing bag-mask device for use:
o select a face mask of the appropriate size (if included)
o connect the mask to the device;
o place the bag-mask device next to the patient.
- Position yourself at the top of the person’s head.
- Open the airway:
o with your hand on the forehead and your fingertips under the point of the chin gently tilt the person’s head backwards, lifting the chin to open the airway;
o hold the person's head in this position with your fingers under the chin.
- Place the face mask:
o With your other hand, grab the face mask at its connector to the bag with fingers 1 and 2.
o Place the mask of the bag-mask device on the person’s face, paying attention to the correct orientation (nasal-oral edge)
o Use the clamp technique to hold the mask in the correct position while you lift the jaw to obtain an open airway. To perform the clamp technique, take the index finger and thumb of your hand and form them into a ‘C’ around the top of the mask.
o Your other fingers of the same hand are used to lift the jaw.
- Squeeze the bag to deliver a breath (each breath should be delivered over 1 second). Watch for chest rise. If you do not observe chest rise, you do not have a tight seal. If this occurs, reposition the mask and try again. Be careful not to overinflate the lungs (each breath should result in visible and natural chest rise).
- Provide 2 ventilations after every 30 compressions.
