OSKE (Objective Structured Clinical Exam)
OSKE is a sequential demonstration at training sites (they are called "stations") of practical skills (elements of professional activity). The exam will use 8 stations that simulate various aspects of professional activity. There are no examination cards, the principle of this form of exam is “everyone passes everything”. OSKE allows one to evaluate and demonstrate what trainees “can do”, not what they “know”. The results of assignments at the stations are assessed using special tools (checklists, rating scales, software and hardware).
Exam procedure
- Students are invited to the exam, usually as part of a study group, are registered and divided into even and odd numbers. One numbers go through 4 stations in room number 1-4, the other numbers go through 4 stations in room number 5-8.
- The student has time, usually up to 30 seconds, acquainted with the task and the conditions of activity at the station.
- A signal is given, the student enters the station and performs the element of professional activity established by the task at this station for a certain period of time, usually 5 minutes. If this is established by the script, upon completion of the work, the workplace is restored to its original state.
- The teacher observes the actions of the examinee, fills out a formalized document (checklist), conducts assessment according to the developed methodology, puts the assessment result in the checklist form. The examiner is prohibited from communicating with the examinee, if this is not provided for by the station's script.
- A signal is given, the student completes the work, regardless of the completeness of the element of professional activity, leaves the station and moves on to the next one. In this case, the examiner stops recording the student's actions, filling in the remaining student actions in the checklist as unfulfilled.
- The steps are repeated until all students in the group have passed all the stations.
Station 1 and 6 "First aid for sudden death (first rescuer)"
Situation
You are in a shopping mall. You see a person suddenly fall to the floor. You are the first person to approach the victim. Provide first aid to the victim
Task:
1. Perform a primary examination
2. Organize first aid
3. Perform indirect heart massage
Limitation:
Time limit 5 minutes
Put on gloves and a mask before entering the station
When entering the shopping mall, you saw the AED sign
You have one assistant to organize assistance
After the procedure, you do not need to take off your gloves (not assessed)
Station 2 and 7 "First aid for sudden death (second rescuer)"
Situation
You are in a shopping mall. You heard a call for help. Approaching the scene, you see the victim lying on the ground and a person next to him. This person asked you to help provide first aid to the victim
Task:
1. Call an ambulance
2. Use the AED
3. Perform artificial ventilation
Limitation:
Time limit 5 minutes
Put on gloves and a mask before entering the station
When entering the shopping center, you saw the AED sign
Artificial ventilation should be performed using a breathing bag
After the procedure, do not remove the gloves (not assessed)
Station 3 "First aid to a victim in an accident"
Station 4 "First aid for injuries"
Situation
You witnessed a worker fall from the 2nd floor of a building under construction. The scene of the incident is safe. The victim is conscious and calling for help. Upon examination of the victim, a wound was found in the upper third of the shin, from which a pulsating stream of bright red blood was flowing.
A tourniquet, a gauze napkin, and a folding splint for immobilizing the limb are available to provide assistance. Provide first aid to the victim.
Task:
1. Perform a primary examination
2. Stop the bleeding
3. Immobilize the injured limb using a folding splint
Limitations:
1. Time limit 5 minutes
2. Put on gloves and a mask before entering the station
3. The means of providing assistance have been delivered and are located next to the patient
4. Communicate with the phantom as with a real patient
5. Imaginary assistants can be used to provide assistance
6. After the procedure, there is no need to remove the gloves (not assessed)
Station 5 "First aid for limb injury"
Situation
The victim, while indoors, slipped and hit his right forearm on the corner of a table. The victim asked you to help him. The scene is safe. The victim is sitting on a chair, leaning on the back, supporting his right forearm with his left hand. Complaints of pain in the right forearm during palpation and movement of the limb. No wound was found during examination of the injured limb. A first aid kit, a folding splint for immobilizing the limb, and a medical sling are available to provide assistance.
Task:
Examine the patient
Immobilize the injured limb using a folding splint and a medical sling
Limitations:
Time limit 5 minutes
Put on gloves and a mask before entering the station
Means of first aid have been delivered and located near the patient
Communicate with the phantom as with a real patient
